This worksheet will be considered the official record for users and partner agencies to submit information for entry into the CAD files. No other forms will be accepted.
Indicate if the information is for Police only, Fire/EMS only, or both.
Information may be placed into a Hazard, Temporary Situation or Direction CAD files.
Hazard File is defined as either people or property that may be hazardous to responders.
Temporary Situation File is defined as information that may be helpful to responders in performing their duties.
Directions File is defined as information that may help a responder locate the scene.
If no date is given, a date 90 days from entry date will be used. Expiration dates can be a maximum of 1 year.
