Basic Life Insurance
The City provides Basic Life and AD&D coverage for eligible employees through Standard Life Insurance Company. This coverage is paid for by the City of Eugene.
Supplemental Life and AD&D Insurance
Employees and/or their spouse/domestic partner may apply for Supplemental Life and AD&D Insurance coverage through The Hartford. The premium is conveniently paid by the employee through a payroll deduction – no check to write every month!
Guaranteed Issue coverage may be available without the need for evidence of insurability during initial eligibility or Special Enrollment periods. New employees and/or their current spouse/DP may apply for a guaranteed issue amount within 31 days of the employee's date of hire. An employee's new spouse/DP may apply within 31 days of marriage/partnership. All other applications require completion of an Evidence of Insurability form and underwriting by The Hartford.
Applications for new coverage or a change in coverage may be made at any time throughout the year. An updated declaration of domestic partnership form may be required. Send completed application forms to Benefits staff.

Public Safety Officer’s Benefit Program
The Federal Public Safety Officer’s Benefits Program (PSOB) provides death benefits to eligible survivors of public safety officers who die in the line of duty. The PSOB program also provides disability benefits for public safety officers who have been permanently and totally disabled by injury sustained in the line of duty.
As defined by Congress in Public Law 90-351 (Sec. 1217), a public safety officer is an individual serving a public agency in an official capacity, with or without compensation, as a law enforcement officer, firefighter, or member of a rescue squad or ambulance crew.
Long Term Disability Insurance
This salary-replacement insurance is provided for regular EPEA, IAFF, IAFF BC, AFSCME and Non-Represented employees who are regularly scheduled to work 20 or more hours per week. IATSE-Represented employee eligibility is specified in the most recent labor agreement between IATSE and the City of Eugene.
